Investigation of Perioperative Anaphylaxis

Release of Information

Before your appointment QARC will gather the relevant medical records relating to your reaction, and sometimes records from prior surgeries, which may need to come from different hospitals. You will need to authorise us to obtain records on your behalf.

In order to accurately diagnose and investigate an episode of perioperative anaphylaxis, every piece of relevant information must be obtained and evaluated. Key information may lie in a previous anaesthetic or surgical record.

When you are referred to us, QARC will seek your permission to obtain prior medical records to assist us in investigating your reaction. 

Some hospitals and equivalent facilities may require an original, signed “release of information” form from a patient. If this occurs, QARC will provide you with the relevant paperwork to complete.
